He died from a series of unfortunate events. The first thing that happened was someone forgot to make sure all the primer's had been fired on the bullets (the powder had been removed). One primer was still unshot. This primer had enough power to lodge the slug of a bullet into the barrel of the gun. The next day when they were shooting, they were using the primer and the powder (but not the slugs) on bullets, to produce the "bang" sound and the smoke.
