Usually when it involved being kneed/kicked in the head repeatedly. And they didn't look bloody. I don't know what causes it. Reading the "sensei's" comments, he said the guy was warned not to come to the dojo because it was not like any other dojos. It's not just a typical "self-defense" class, but a class where people regularily got their ass beat. He said the black belt took so long taking out the retarded dude, that he got his ass beat for a week as punishment.
