Best one I've ever had was at some gourmet burger place that my cousin took me to. It had applewood smoked cheddar and apples, which sounds bizarre but was unbelievably good.

 

had the same thing once at a steakhouse. Angus beef with smoked applewood chedder and bacon. Delicious as hell

Said to be the oldest hamburger restaurant in the U.S.- Louis Lunch in New Haven, CT. They still use original equiptment. According to them: "Cheese, tomato and onion are the only acceptable garnish -- no true connoisseur would consider corrupting the classic taste with mustard or ketchup."
